The lecture presented by Dr. Craig M. Misch was part of the 34th Annual USC International Periodontal & Implant Symposium held in Los Angeles in 2009. Dr. Misch received postgraduate certificates in prosthodontics and oral implantology and a Master of Dental Science degree from the University of Pittsburgh. He then completed specialty training in oral and maxillofacial surgery in Pittsburgh. Dr. Misch holds dental faculty appointments at New York University, the University of Florida and the University of Pittsburgh. He is a Diplomate of the American Board of Oral and Maxillofacial Surgery and currently practices as a dual specialist in Sarasota Florida. He is on the editorial boards of several professional organizations.

The risks shown in this short animated film may appear with every wound.
They arise even more often than the special complications (which are explained in our other film), and normally they don´t last that long.
Mostly after removing a wisdom tooth the patient feels pain. It lasts normally some days and with pain killers it is easily controllable.
The pain uprises through irritation of the ends of the nerves in the operation wound.
We try to reduce the pain by removing the bone very slowly and with less pressure.
The wound healing is suspicious, if obvious complaints crop up again suddenly after a painless phase of some days. A swelling of the jowl occurs almost ever and it even intensifies until the second day after the operation.
After one week it has mostly already disappeard.
Heat sinks may reduce the swelling but also the pain.
Actually, behind the swelling burrows an useful clearance of the body.
The body must get the defending cells out of the bloodstream into the injured tissue.
Then the bloodstream is slowed down, the jowl warms up.
The vessels become permeable and the defending cells can get into the tissue.
Together with the defending cells also liquid penetrates into the tissue and it leads to a swelling.
Finally the subject inflammation: normally after having sutured the wound, it is rebuilt into new bone.
But especially this area is washed around with leftovers which might also stay in the wound.
An inflammation appears classically after three weeks after the intervention.
The hole in the bone gets pus-filled.
The patient recognizes once again a swelling of the jowl and because of the pus a bad taste in the mouth.
On the other hand, the pain is comparatively less.
Normally the treatment is easy by ensuring, that there is enough outflow out of the wound.
If the wounds are endangered, we always put in a tamponade immediately which guarantees the draining off.

Normally removing wisdom teeth is practice. But even there complications may appear. At the moment we are producing a trick shot about such complications and we ask the youtube audience in this stage for their comments.
A possible complication is the damage of the neighbouring tooth. This one may be squeezed and damaged during removing. In case of doubt, we try to separate the tooth, so that the single parts can be taken out safer.
Another complication may be the damage of the nerve of the lower jaw. It leads stimuli from the lip to the brain. This risk is much higher, if the roots of the tooth go very closely to the nerve. The risk and how to avoid it. you can understand the best by means of a sectional drawing. The nerve is located in the middle of the lower jaw. The wisdom teeth are usually at the side of it. It gets more difficult if the roots of the wisdom tooth are bended and twisted in the direction of the nerve. If such a tooth is removed, the nerve may be damaged. Even more demanding is it as soon as the roots grab around the nerve, similar to small hooks. Then the tooth is separated and the single parts are removed. Therefore, we use fine ultrasonic drillers and enlargers like the operation microscope.
Another important structure is the nerve of the tongue. lt passes by an the inside of the mandible under the mucosa. Often this nerve is far away from the operation wound.
But sometimes the wisdom tooth is located at the inside of the lower jaw. Even the nerve of the tongue may pass by in an unusual high position. In this case the driller comes very closely to the nerve. To avoid a contact between the driller and the nerve of the tongue we put in a special instrument for protection.
With this film we wanted to show you the most relevant risks that are specific with removing lower wisdom teeth.
Finaliy we want to emphasize, that permanent complications alter having had such a routine intervention are fortunately very seldom and furthermore it is an important duty for the operating surgeon to reduce the risk as well as possible.
